Dupage County’s Local Rule 5.06(b) allows users to e-file exhibits as either part of an envelope or separately. If a user does decided to file it as part of one envelope, then the will combine it with the lead document PDF. No Exhibit Cover Sheet is needed in this instance.

However, when filing the exhibits separately, users need to complete and add the Dupage Exhibit Cover Sheet, Form 4393, as a cover page for each exhibit. The form can be found on page 3 of the following PDF document. If you do not use the Exhibit Cover Sheet the clerk will reject the filing, and request the filer add it.
